How Does Shockwave Treatment Work?

Shockwave therapy is a popular non-invasive treatment using Radial Pressure Wave (RPW) technology to transmit acoustic shock waves into the pain region through a specialised transmitter and handpiece, reducing pain and stimulating healing.

The shockwaves stimulate healing at a cellular level to repair damaged tissue. The treatment increases blood flow and promotes the growth of collagen and new blood vessels, also known as neovascularisation ingrowth. It also reverses chronic inflammation and breaks up any calcium build-up to create the best environment for healing, returning the injured area to normal and restoring functionality.

The shockwaves also influence the body’s pain-regulating mechanisms to provide immediate local pain relief.

How Is the Treatment Applied?

Assessment:

  • The physiotherapist conducts a thorough assessment to determine if the patient is suitable for shockwave therapy.

Preparation:

  • The area to be treated is exposed, and a conductive gel is applied to ensure efficient transmission of the shockwaves.

Application:

  • A specialised portable handheld device administers shockwaves to the affected area. The strength and rate of the shockwaves depend on the patient’s tolerance and the particular condition.

Duration:

  • A typical session lasts between 10 to 20 minutes, depending on the condition and the treatment area.

For the best results, it is usually recommended that the patient have 5 to 10 sessions to fully rehab the injury.

While minimal discomfort may be experienced during the treatment, most patients notice an immediate reduction in pain around their problem area within the first twenty-four hours.

What Can Shockwave Therapy Help With?

Clinical studies have shown marked improvement in the following conditions using shockwave therapy:

  • Myofascial Trigger Points: Localised tender or painful areas.
  • Plantar Fasciitis: Inflammation of the ligament across the bottom of the foot.
  • Chronic Tendinopathies: Swollen, painful tendons such as:
    • Tennis/Golfer’s Elbow (lateral epicondylitis)
    • Achilles Tendinopathy
    • Shoulder Tendinopathy & Rotator Cuff Issues
  • Gluteal tendinopathy: Hips and Buttocks
  • Insertional tendinopathy: Where the tendon meets the back of the heel bone
  • Patella tendinopathy: Injuries to the tendon that connects the kneecap
  • Proximal hamstring tendinopathy: Where the tendons connecting the hamstring muscles to the pelvis become irritated or damaged, causing pain and stiffness in the upper thigh or buttock area.

Shockwave therapy is also very effective in treating common musculoskeletal problems such as shin splints, muscle strains, calcified haematomas, and Osgood Schlatter Disease. Compared to other conventional treatments, shockwave therapy treats the musculoskeletal pathology rather than just relieving symptoms.

Physiotherapy & Shockwave Therapy

Shockwave therapy, when combined with physiotherapy, is a comprehensive way to fully recover from a primary injury and help ensure it does not weaken and reoccur in the future. Shockwave therapy will treat the injury directly, and physiotherapy will strengthen the supporting joints and tissues while addressing other issues that may worsen the pain.

Frequently Asked Questions

What are Shockwaves?

Acoustic waves or sound waves with very high energy bursts you see with lightning strikes are known as shockwaves. When using shockwave therapy for treating chronic pain, the penetrating sound waves directly target the body’s tissue layers, creating the optimal environment for healing.

What is the difference between shockwave therapy and ultrasound?

Ultrasound uses multiple-pulse sound waves to heat body tissue, while shock waves use high-energy single-pulse sound waves to stimulate natural healing.

Is Shockwave Therapy suitable for everyone?

Shockwave therapy is not suitable for everyone. Precautions include:

  • Pregnancy
  • Disturbed sensory and nervous function, e.g., Diabetes
  • Recent corticosteroid injections (wait at least 6 weeks after local injections)
  • Malignancy
